- Deadline Date for Pre-Registration - May 1, 2023.
- Late registration will only be accepted if there is space available.
- There is a $625 registration fee due by May 1, 2023.
- There will be a $50.00 late fee for all registrations received after May 1, 2023.
- There will be a $75.00 late fee for all registrations received after June 1, 2023.
- There is a 20% handling fee on all refunds made prior to June 1, 2023.
- No refunds or credits are permitted after June 1, 2023.

MEDICAL AUTHORIZATION

I, Parent First Name* Parent Last Name* , am the parent and legal guardian of Student First Name* Student Last Name* a minor child (under the age of eighteen (18) years of age) give my permission to Dance Masters of America, Inc., the Officers of the Organization, Directors and Assistants of the Student Honors Intensive Program to obtain whatever medical and/or hospital care and treatment may be deemed necessary while on-site of classes from July 29th through August 3rd, 2023.

DANCE MASTERS OF AMERICA S.H.I.P. ATTENDEE AGREEMENT
In consideration for being accepted into the Dance Masters of America Student Honors Intensive Program the undersigned, jointly and severally, do hereby release Dance Masters of America, Inc., the Officers of the Organization; the Directors and Assistants of the SHIP Program; the faculty; The State University at Buffalo, New York, from any and all claims for damages, or for injuries which the Attendee may sustain while participating in any and all activities with the Dance Masters of America S.H.I.P. program.

*A DOCTORS NOTE stating the attendee is fit to actively participate in the program is REQUIRED WITH THIS APPLICATION.

SPECIAL NOTICE:
- This program is for dancers ages 13-18 years of age.
- All late registrations will be subject to space. Students not registered by the deadline date of May 1st must call the National Treasurer at 504- 454-1376 to check availability.
- All Attendees must include a doctor’s letter with the S.H.I.P. registration form stating that the attendee is fit to actively participate in the program.
- Attendees should fly into greater Buffalo International Airport.
DRESS CODE:
- Female:
  - One-piece leotards, (no two-piece dancewear or undergarments showing) Footed, footless stirrup tights or black leggings
  - Cover-ups: Permitted only for the beginning of class.
  - Appropriate shoes for each class.
  - Hair must be secured neatly.
  - Female students must bring a basic black leotard.
- Male:
  - Jazz pants, tights, or form fitted shorts and t-shirt Dance belt
  - Appropriate Shoes for each class Hair must be secured neatly.
  - Male students must bring basic black t-shirt.
*All students must bring your basic black leggings or basic black jazz pants for showcase performance.
