    Trustee Information

    Including Declaration of Eligibility
  • This form is required for all Charity Trustees. Information provided is treated as  private and confidential and will only be shared with authorised staff or as required by law.

We are also required to register you as a Trustee on our record held with the Charity Commission visible to the public. Only your name and position on the committee is shown - contact information is not visible. Once registered with the Charity Commission, they will send you an email/link to set up your own Trustee account.

We are also required to share details of Trustees with some other organisations we deal with (eg, our banks and accountants), so they may carry out ID-checking and verification, as required by law. For this, they will request from us or you directly a valid form of ID (eg driving license, passport) and proof of address(es).

If you are unable to complete this form digitally on your phone, tablet or other device, we can provide a paper version to complete instead.

  • Due Diligence / Verification of ID

Due Diligence / Verification of ID

  • IMPORTANT - As already stated at the top of the form, due diligence checking is carried out by some organisations we deal with to verify the identity of individuals holding key positions in charity organisations. Usually this refers to financial organisations such as banks and card payment providers. For this purpose, we are required to share personal details (name, address and date of birth), IDs and proof of address with those organisations when requested.

Note - ID verification can be key to other processes or services being maintained for the charity (eg bank accounts remaining active). Therefore, please let us know if you think there may be an issue with providing any form of ID.

  • For convenience, you can upload ID and proof of address documents below or omit this section and provide them if and when requested by the relevant organisation. Also, some organisations send links to individuals allowing them to upload key documents directly.

  • If you provide proof of ID - eg passport or driving license -  the scan or photo must show all corners of the document.

  • If you provide proof of address, it cannot be your driving license if that has been used as proof of ID. The proof needs to be dated within the last 3 months - in which case you may be asked again if the document expires.
