This form MUST be completed PRIOR to scheduling your move-in appointment.
Utilities MUST be transferred to your name no later than your scheduled move-in date. Not switching utilities will delay the release of keys to the unit. If a utility does not apply to your property please fill N/A in the blank. Any utility bills sent to property management company AFTER move-in date will be billed back to the tenant along with a $75 non-compliance charge billable each month until the utility information is corrected.
