I hereby authorize the staff of Ram Camp and Ram Destinations or its designee, into whose care the camper has been entrusted, to consent to any X-ray examination, anesthetic, medical or surgical diagnosis, treatment, and/or hospital care to be rendered to the camper upon the advice of any licensed physician and/or dentist. It is understood that this authorization is given in advance of any required diagnosis, treatment, or hospital care and provides authority and power to Ram Camp, a division of Ram Destinations, to grant specific consent to any and all such diagnosis, treatment, or hospital care which a licensed physician or dentist may deem necessary. I know that all costs of paramedic transportation, hospitalization, and any examination, X-ray, or treatment provided regarding this authorization shall be my sole responsibility as the camper's parent/guardian. I understand that Ram Destinations, Ram Camp, its officers, and employees assume no liability of any nature in relation to the camper's transportation. I authorize Ram Camp staff to sign waivers to trips if I send my child on a trip and forget to sign the required waiver.I also understand and agree that Ram Camp and Ram Destinations have the right to use pictures and videos of campers and activities at camp and on the program for promotional purposes, including online and offline.I further acknowledge that I will not hold Ram Camp, Ram Destinations, its officers, or employees responsible for any accidents or injuries that may occur as a result of normal camp activities and normal interactions between children.*
