MEDICAL PERMISSION

By submitting this electronic registration, I give my permission to CSz Richmond Theater to administer any necessary first aid to my child in case of emergency and, if warranted, to take my child to an emergency clinic or hospital. I understand that CSz Richmond Theater will contact me to inform me of any incidents.

In consideration of my participation in the activities of CSz Richmond Theater, I hereby fully release and discharge, and save whole and harmless, ComedySportz Richmond, ComedySportz Improv Theatre, CSz Richmond Theater, Yes Balloon, their owners, officers and employees, their successors or affiliates from any and all liability for damages or claims for damages, causes of action, claims, demands, costs, expenses, and compensation of any nature whatsoever, for any and all known and unknown personal injuries, sickness, illness, or disorder, which I may now hereafter have, arising out of or in connection with participation in any activities whatsoever of ComedySportz Richmond, ComedySportz Improv Theatre, Yes Balloon, or CSz Richmond Theater.

I give my permission to CSz Richmond Theater to take photographs of campers and camp activities for the purposes of promotion and marketing of CSz Comedy Camps for future use. PRICING

One-week Camp ComedySportz® camps are priced as follows:
- $275 when you register by February 15.
- $300 when you register by by March 31.
- $325 when you register April 1 or later.
Submitting this form will direct you to a PayPal page for the required $50 deposit for each camp selected per camper. Please submit this form separately for each camper attending. Your balance will be due by the start of the camp.
