The purpose of this form is to get a concise idea of what you'd like to be tattooed, so please answer all questions and fields carefully and thoroughly!! Once I am able to review your submission, I will email you to work out the rest of the details.

IMPORTANT INFORMATION

✶ My minimum price per piece is $120

✶ My hourly rate is $150+HST for larger, long-term projects

✶ Smaller projects are priced on a piece-by-piece basis, depending on sizing, intricacy and placement

✶ A non-refundable deposit of $50 - $150, depending on the project, will be required to book an appointment - please do not send any money until you are asked for a deposit!

✶ My working dates are Wednesday - Saturday (Saturday appointments are limited)

✶ If you have multiple larger/more complex tattoo ideas, please submit them all in their own separate form, as I find it's easier for me to stay organized that way! If you have multiple small ideas, they can be combined on one form, make sure to include the info for each piece in the prompts

✶ Please keep in mind it can take up to 2 weeks to hear back from me, as I'm just one person who's also preparing designs for appointments and tattooing 4 days a week - thank you in advance for your understanding!

At Witch in the Woods Tattoos we strive to make everyone feel welcome and comfortable during their sessions. While we realize we can never make everyone happy, I would love to know if there is anything specific we can do for you to make your experience more comfortable.

At the moment we use mobile privacy screens for clients when necessary, but we are hoping to have curtains around each station that can be used to make sessions even more private in the near future.

We currently have two full-time artists, so there could be another client in the studio during your session, for this reason, we ask that all clients ask us permission before bringing a guest with them, to help respect the privacy of other clients.

Deposits
A NON-REFUNDABLE DEPOSIT OF $50-$150 IS REQUIRED TO BOOK ANY APPOINTMENT. This comes off the price at your last session. This deposit is to ensure that you are serious about booking your appointment and will be deducted from the total cost of your tattoo.*

If your submission is accepted and you decide to book, you are booking for THIS tattoo idea, if you decide to change your idea completely you will have to submit a new form, and will require a new deposit to book the new idea. Deposits are non-refundable, please be sure you want to book before you send your deposit*

Rescheduling
We require a minimum of 72 hours notice to reschedule your appointment. If you choose to cancel your appointment altogether, your deposit will be lost. Please let us know ASAP if you cannot make your appointment so we can reschedule you without affecting your deposit. ** there are exceptions to the 72 hr notice ie. snowstorms and road closures*

We allow for two reschedules, if subsequent rescheduling is needed, a new deposit will be required. If your artist reschedules the appointment, this is not included as one of your reschedules.*

Notice to cancel or reschedule given on the same day as your appointment will be treated as a No Show, you will be charged a No Show fee of $250 and lose your deposit. A new deposit will be required to reschedule (Please just give us notice, we want to be kind and don't want to charge anyone this fee, but if we're not given enough time to fill the space, we will be forced to, please remember this is our only source of income, and if we’re not tattooing, we’re not earning)*

Appointment Time
Please do not show up more than 15 minutes EARLY OR LATE for your appointment. Please contact us via email if you are going to be late. Depending on the appointment time allotted, being more than 20 minutes late to your appointment MAY result in the forfeit of your deposit and the cancellation of your appointment (this is usually only an issue with smaller appointments, but please be aware and give us a heads up if you're late). A new deposit will be required to re-book any appointment that was canceled for lateness. Deposits are non-refundable.*

Tattoo Design
All tattoo work is custom. WE DO NOT DO DRAWING VIEWINGS AHEAD OF TIME (in person or over email). There is always time for minor changes/adjustments on the day of your appointment. If you are unsatisfied with the entire design or want to move in a different direction completely please understand that in doing so the deposit is forfeited for time spent drawing, and planning. PLEASE BE THOROUGH IN YOUR EXPLANATION SO THE DESIGN REFLECTS YOUR IDEA AS WELL AS POSSIBLE*

Touch-Ups
We guarantee the quality of our work (except for hands and feet due to the high rate of ink fallout with daily wear and tear & rapid cell regeneration - touchups on these areas are charged at $120+HST), however, people heal differently, and damage their pieces with scratches or other circumstances. Please be sure to follow the Aftercare Instructions given. If you feel like you may need a touch-up, please send a photo of your tattoo to Emm. The touch-ups must be completed within a year of your initial tattoo date.*

By signing (draw your signature) below, I certify that I am at least 18 years of age. I have read this tattoo agreement and fully understand the contents thereof.
