Here is the list of documents needed:

• ID and Social Security Card
• Pay Stubs (Both you and your spouse)
• Profit/Loss Statement for any businesses you own
• Award Letter: Food Stamps, Social Security Income, etc.
• Bank Statement (Both you and your spouse, limit accounts to 2 per person)
• Retirement Statement (Pension, etc)
• Tax Return (Both you and your spouse)
• Divorce Decree
• Mortgage Statement
• Auto Loan Statement
• Apartment Lease Agreement

REMINDERS:

1. The PDF file size must not exceed 15MB.
2. You can only upload a PDF file using this form.
3. The upload field is set as required which means you will not be able to submit the form without uploading a file.
