Cancellation & Change Policy

To limit the negative impact of last minute cancellation and changes to other customers' camp experience, cancellations and changes are subject to the following fees:

More than 10 days	$10 fee per camp	75% refund
Less than 10 days	$50 fee per camp	50% refund

PARENTAL CONSENT, RELEASE OF LIABILITY, AND INDEMNITY AGREEMENT

In consideration of services performed by Funtastik Labs, LLC., a Texas Limited Liability Company, who is owner and operator of Funtastik Labs, at Funtastik Labs today and in the future, I, in my legal capacity as the parent/guardian of the minor named below, hereby execute this Parental Consent, Release of Liability, and Indemnity Agreement (the “Agreement”) on behalf of myself and any children, relatives, or dependents identified herein (collectively ‘Participants’), and hereby agree as follows:

I and Participants shall abide by all Funtastik Labs rules, instructions, and the directions of Funtastik Labs employees and representative. Failure to abide by those rules, instructions, and directions can lead to the immediate revocation of my rights to use Funtastik Labs services and facilities, without any right to refund or any payment.

I understand, FUNTASTIK LABS IS NOT LICENSED WITH THE STATE OF TEXAS AND IS NOT CONSIDERED A CHILD CARE FACILITY, A DAY CARE FACILITY, OR AN AFTER-SCHOOL PROGRAM.

I and Participants grant exclusive permission to Funtastik Labs to use my or Participants’ names, faces, likenesses, voices, artworks, activities, presentations, creations, or appearances in photographs or videos for educational and promotional purposes such as, but not limited to, brochures, flyers, website, and social media without compensation and without restrictions as to frequency and duration.

In the event of a medical emergency, I and Participants authorize Funtastik Labs, LLC. to transport my children to a hospital or emergency room for medical treatment and hold Funtastik Labs, LLC. harmless in the execution of such. I agree to be responsible for any associated transportation and medical expenses.

In consideration for being permitted access to Funtastik Labs facilities and activities, I and Participant voluntarily assume all risk of injury from participating in Funtastik Lab activities and agree to fully and forever waive, release and discharge Funtastik Labs, LLC., its affiliates, owners, members, investors, representatives, employees, instructors, contractors, landowners, landlords, affiliated companies or entities, successors, heirs, and assigns, of any liability or obligation that may arise out of or in connection with my activities or the activities of others while at Funtastik Labs.

Funtastik Labs does not warrant any success rate or outcome with respect to participation in Birthdays, Camps, Open Lab, Clubs/Classes, Workshops, or any other activities and events.

By executing this agreement, I declare under penalty of perjury that I am doing so only for myself and/or Participants for whom I am authorized. If I or Participant or any legal representative files a claim for lawsuit out of my or Participant’s participation in Funtastik Labs, I agree to defend, indemnity and hold harmless Funtastik Labs, LLC.

I understand the terms of this PARENTAL CONSENT, RELEASE OF LIABILITY, AND INDEMNITY AGREEMENT and understand that this release of liability will prevent me and Participants from filing suit or making any claims for damages in the event of any loss or injury arising from participation in Funtastik Labs activities.

I have carefully read this agreement and fully understand its contents and unconditionally agree to it fully and voluntarily.
