Complete Participant Release Form

Before starting the registration process, collect all "Participant Information and Release Forms" for youth, coaches, and volunteers associated with teams you plan to register.
1. Download the PDF below.
2. Distribute to participants and/or guardians BEFORE continuing with registration.
3. Collect completed and signed forms. Scan or take photos of completed forms (both pages!).
4. Complete registration and upload completed forms for ALL participants on final page of this form.
Regardless of school policy, all individuals participating in this year’s Envirothon (youth, coaches, volunteers, etc.) must submit completed "Participant Information and Release Form" at time of registration.
