Dr. Rob Faulkner is happily accepting new patients to our practice. We do however want to make sure you are aware of our office policy prior to making an appointment so that we ensure everything is good to go and ready once you are scheduled with us.
We are out of network with all insurance companies, however we can print out a claim for you to submit to your insurance for reimbursement and they will either approve or deny the claim. This does mean that your appointments will all have to be paid out of pocket.
- For the most accurate results for every patient, we typically have 3 appointments needed prior to starting treatment:
- First appointment will be the comprehensive exam where the Doctor will look over any x-rays and your medical/dental history, look into your mouth, check out your throat and neck as well. This appointment is $285
- Second appointment we get any impressions and measurements of your mouth and jaw plus any additional x-rays that may be needed. This appointment is $421.
The third appointment is the consultation where he will go over your options for treatment based on his findings, and he will thoroughly explain what he sees that will need done to ensure you are getting the option that will be most effective and give the best results overall. This appointment is $213.
These costs for new patients are NOT including any x-rays we may have to take. However, if you have any xrays from a previous dentist’s office it would be very helpful to have them send them to our office at implants@cpordent.com so we can evaluate to make sure if we would even need to take any additional x-rays.
As far as payment goes; we accept Cash, Check, and Card as forms of payments. If cash or check is used at time of payment there is a 3% discount. If you bring a copy of your insurance card so that we can have that on file and ensure everything will be sent to the correct location. If you would like us to fill out a claim form on your behalf for your insurance please bring all pertinent information such as:
The name of the company you or your spouse work with (if retired, state that you are retired)
The name of the insurance company
The address that the claims will need sent to
Your Group ID number and the Primary member ID (some insurances do not provide member ID and use SS#)
