This questionnaire is designed to help Anderson County Shelter Friends, Inc. DBA Bourbon Tails Cat Café, and their rescue partners, find the best possible home for our adoptable cats and kittens. All adopting parties are required to complete this form prior to adoption. ACSF and their rescue partners reserve the right to deny any adoption application based on answers to this questionnaire and/or interviews prior to adoption. Please be honest and transparent with your answers so we can make sure you adopt the cat that will best suit your lifestyle. To ensure out of town adopters are able to leave with their cat the day of their appointment, please fill out the application 48 hours before appointment time. 

Please notify your Veterinarian(s) that you give them full permission to share past and present client information with Anderson County Shelter Friends, Inc and/or their rescue partners.

ACSF and rescue partners attempt to give an honest evaluation of any cat based on our assessment or that of the source. Should you encounter any behaviorial or health concerns, please contact us so we are able to work together to resolve the issues. 

ACSF and rescue partners have completed necessary vetting on all cats and kittens residing at the cat café and administer flea/tick and wormer medications on a routine basis recommended by our veterinarians. Even with this regimen, we cannot guarantee that any cat/kitten adopted will not show signs of fleas, ticks or worms after adoption and will not be responsible for vet bills following the completion of adoption.

All cats are non-transferable and must be returned to us if you, the adopter, are no longer able to provide care. All cats being returned must have a scheduled intake appointment so ACSF and rescue partners can accommodate for space. Space may not be readily available and you, the adopter, will be required to hold until space is open. This could take up to four weeks. Adoption fees are refundable for 30 days after adoption. 

ACSF and rescue partners adhere to a strict INDOOR ONLY and NO DECLAWING policy unless discussed otherwise. All applications are evaluated without bias. We evaluate based on the needs of the cat. A completed application is not a guarantee of acceptance. Any falsehoods will be subject to automatic disqualification. We match cats with the most suitable adopters not based on a first come, first serve basis.

Thank you for opening your hearts and home to one (or two!) of our amazing adoptables! We appreciate your interest and ask for your patience as we work through the applications. Please know that out of town guests to the Café will be required to fill out an adoption application two days prior to your cat lounge appointment to ensure you get to take your cat home same day. If an application is not filled out 48 hours beforehand to allow time for approval, you may be asked to come back. You may contact us at info@bourbontailscatcafe.org for updates. Please notify us if you adopt/foster elsewhere if your application is still in progress- we'll just be glad you're helping a kitty regardless of where it came from!

I understand that I assume full responsibility for the welfare of this pet(s) from the date of adoption and that failure to provide said welfare can result in termination of ownership rights and said cat(s) will then return to Anderson County Shelter Friends, Inc. DBA Bourbon Tails Cat Cafe.

By submitting this application, I affirm that what I have submitted is truthful to the best of my knowledge and complete. I also understand and agree to the adoption policies and agreement as outlined in this document above.
