    I acknowledge that by checking the "yes" I agree to the terms of the following statements
  • Members are entitled to one facial per month, fully customized for their facial needs, plus additional member benefits.
  • The initial membership period shall be for six months to see initial monthly facial benefits. A member shall not be entitled to cancel their membership within the initial period. After the initial period, membership may be cancelled with a 30-day notice, or will continue as month-to-month unless otherwise specified. If you decide to re-active your membership, another six month commitment is required.
  • You are responsible for scheduling your monthly facial treatment in advanced to secure the desired time frame. If you are unable to attend, cancellations must be made 24 hours prior to appointment time or the service shall be forfeited.
  • You are able to freeze your membership for up to two months, once per year. You also have the option of gifting that month’s facial to a friend or family member two times per year. Other than previously specified, your membership is non-transferrable and non-refundable.
  • Monthly membership credits must be used within the 4-6 week period and do not roll over. $15 facial voucher may only be used by the same person twice in one year.
  • Monthly payment will be charged to the card on file in your client profile starting from the day you start your membership OR at every appointment as long as it is pre-booked at the time of each appointment. If you'd like to switch payment for the month, please email info@houseofaloeslc.com If your payment is unable to process, the membership will be put on hold until payment is updated.
  • I acknowledge gratuity is not included in my monthly membership package.
