Thank you for your service to the Bethel Evangelical Baptist Church family and to our community as we seek to fulfill the great commission and live out the great commandment. As we aim to excell in all we do to honor God through an impactful ministry, we must continually evaluate our effectiveness and identify our strengthes and growth areas.

Please complete this form to evaluate the past year and plan for the upcoming year for your ministry area. The first part focuses on your performance as a ministry leader. The second part focuses on the specific ministry you lead. If you lead more than one ministry, please complete this form for each.

Once received, the pastor or a member of the pastoral staff overseeing your ministry area will meet with you to review and discuss this assessment in view of being effective
