Angel Wings Dog Sanctuary Adoption Application

Angel Wings Dog Sanctuary is an all-volunteer, non-profit, animal welfare organization dedicated to the rescue and placement of animals. Angel Wings Dog Sanctuary is completely funded through fundraising efforts and public donations. All of our pets are cared for by foster homes and receive complete medical care.
This questionnaire is designed to help Angel Wings Dog Sanctuary and yourself determine if a specific dog is the right one for you and your household. All adopting parties are required to complete this form prior to adoption. Angel Wings Dog Sanctuary reserves the right to deny any adoption based on answers to this questionnaire and/or interviews prior to adoptions!
Applicant Information

*Please allow 3-5 days for application to be reviewed* ‼️❤️ “Due to the increased costs of veterinary care, we need to adjust our adoption fees. The new fee of $550 covers the animals' exams, rabies vaccines, distemper vaccines, 4DX heartworm testing, flea medication, dewormer if needed, spay or neuter, and any other medical assistance the animal may require. Even with this increase, we still end up paying out of our own pockets a lot of the time. If the animal is under 6 months of age, they cannot be spayed or neutered, and this needs to be done by the adopter. We do offer a $50 reimbursement with proof that the surgery was done. There is a very strict contract on our spay or neuter program which will be gone over during the adoption process. This change will help us maintain the high standard of care our animals deserve. We appreciate your understanding and continued support in finding these pets their forever homes." ❤️‼️
