Canine Good Citizen And Rally Class Registration

Thank you for your interest in signing up for dog training with MP Dog Training Academy, LLC, I look forward to working with you.  During class we will go over the basic obedience with a focus on Canine Good Citizen Behaivors and Rally.

In addition to the CGC classes, this session we are offering AKC Rally. In AKC Rally you and your dog navigate a course, side-by-side, through 10-20 different exercises to show your basic obedient teamwork skills. All classes are designed to build upon the clear consistnt communiction between you and your furry family members.

Class sign up options with details are below. Classes will be held at the Village at Vistancia, 29701 N. Sunrise Point, Peoria, AZ 85383, primarily in the outdoor setting areas.

We allow up to two handlers per dog. All primary handlers must be an adult or at least 16 years of age. Children under the age of 16 are not encouraged for handlers. Please contact Mary (480) 285-9063 for special circumstances.  

Please arrive with your dog ON leash. The leash used for class should be 4ft to 6ft long. No retractable leashes allowed. You may use a harness or gentle leader if that is what you have been using. We might have some better suggestions for you to utilize. 

Please bring treats for your dog. You may bring water or a small backpack of things you feel you may need when out with your dog. Try to limit what you bring as we will be moving around and working in different areas with your dog; so best for you not to have to lug stuff around. 

This class will be a standing class. You are welcome to sit on the ground at anytime but we will not have chairs.  We will be outdoors either all of the time or most of the time.  

Please stay 6 ft apart when you arrive at the registration area with your dog.  ALL dogs are to remain apart, we will have many dogs that have never met before, we want to keep everyone safe and the dogs as calm as possible.  Naturally their will be excitement for them when they see each dog arrive.  Trainers will be there ahead of time to direct everyone.  Let's have fun and stay safe!

Your dog will need to get along with other dogs to attend this class.  Please contact Mary at 480-285-9063 if you have any questions or concerns about your dogs behavior before signing up.  Even though we will be social distancing at 6 feet, your dog still can not have any agressive tendencies towards other dogs.  We would be happy to assist you one-on-one in private lessons if your dog has behavioral issues with dogs.  

You will need to fill out a registration for for each dog separately. If you plan to enroll more than one dog please go through the process a second time.  We require one handler for each dog. One person can not handle two dogs. 

Liability Waiver Terms and Conditions *

I understand that dog training is an activity that is held in the presence of other dogs and their owner(s)/family member(s)/handler(s). I also understand that my participation and that of my dog(s) in dog training may require some physical activity on my part and on the part of my dog. I understand that participation in dog training may include activities requiring phusical exertion or avoiding obstacles, any of, which could result in injury to me, my dog, or others. On behalf of myself, my estate, my hander, and heirs, I understand, agree, represent, and warrant:

1.      That dog training may involve risks to me, my handler, or my dog(s). I knowingly and voluntraily assume all risks associated with participating in dog training lessons and will hold harmless MP Dog Training Academy, LLC, its owners, directors, empoyees, agents or any oher persons or associated entities ("Representatives") in the event of injury to me, my handler or my dog(s). I agree to comply with the instructions, rules and decisions of the training instructor as it relates to me or my dog’s ability to safely complete each training course.

2.      That my health and physical condition allow me to participate in the physical activities necessary to conduct dog training classes.

3.      That my dog(s) is/are: (i) in good health and in a physical condition to participate in the physical activities necessary to conduct dog training classes, (ii) free of any infectious diseases, and (iii) current on all appropriate vaccinations, including Bordetella, Distemper, Parvovirus, and Rabies.

4.      That I will always keep my dog(s) under control and to not allow my dog(s) to interfere with other class participants or activities.

5.      That my dog(s) is/are friendly and not a hazard to persons or other dogs. I agree to comply with any requests by my instructor if he or she feels that my dog is a hazard to persons or other dogs.

6.      That I agree to assume full responsibility and liability for any injury or damage resulting from actions of my dog(s), myself or my handler. I further agree to reimburse any person for any damage that I, my handler, or my dog(s) cause(s) to any other person, dog, or equipment, whether personal or business property.  

7.      I grant authorization and unrestricted license to MP Dog Training Academy, LLC to use my pet’s photograph(s) and/or video(s) taken during training sessions for educational and promotional purposes in any type of media without compensation.

8.      MP Dog Training Academy, LLC reserves the right to refuse training to any dog for any reason. Additionally, I agree that non-compliance with training recommendations constitutes non-liability to MP Dog Training Academy, LLC.

9.   The undersigned on behalf of himself/herself or any owner/handler participants authorized or permitted by the undersigned to attend any lessons, agrees to defend, indemnify, and hold harmless MP Dog Training Academy, LLC, and its Representatives, from all liability and damages for any claims, loss, injury, illness, or expense incurred by any person, animals, or property arising from the undersigned's participation in dog training, consultations, or lessons.

There will not be any refund for classes you can not attend, unless the trainers have canceled class due to weather.  If you can't attend, please let us know and we will see if you can attend an alternate class, but we can not guarantee that.  Thank you
