  • Illumination Tutoring and Counselling 

    I am a counsellor specializing in various counselling methods. I value the  relationship with my clients and believe that such relationships are a vital part of the healing process. 

    I believe that each individual is unique and has his/her own way of addressing problems.  I believe in a wellness model that helps my clients empower themselves by focusing on what works for them and not in a systematic approach that provides a generic procedure for treatment. Each child has there own set of problems to work through so they have their own journey.

    I believe that my clients have the desire and capacity to grow towards fulfilling their true potential under my guidance and that they are experts on their own lives. As such, I will not give advice or offer solutions, but will work with you to help you understand yourself more fully and to find your own ideas and answers. I will also help you develop skills and strategies so you make positive changes. With greater self awareness and trust in yourself, I hope that you will be able to make constructive changes, leading to a more satisfying and meaningful life. If you have service needs beyond my practice scope, i will endeavor to help you identify appropriate referral options.

  • Client's Rights

    1. The client may ask questions on what to expect during and end result of the therapy.
    2. The client may decline to proceed with the therapy as to the techniques which may be conducted by the therapist.
    3. The client may cease to continue therapy anytime, without any impediment and may return to therapy anytime.
    4. The therapist has the right to dismiss the client from the course of therapy.
    5. The client has the right to review his or her records from the therapist.
    6. Right to confidentiality: Within limits provided for by law, all records and information acquired by the therapist shall be kept strictly confidential in accordance to the principles of a doctor-patient relationship. All information will not be shared or revealed to any person, agency, or organization without the prior written consent of the client. Although according to the  harm principle, if there is a situation where there is domestic violence, self harm or harm for others, therapist has the right to contact the necessary institutions. 
    7. Referrals: If client has a particular concern needing a specialized treatment approach, the therapist has the right to refer the client to an other specialist in the field.

    8. The client can raise any concerns and to speak with the therapist immediately of any concerns provided that the therapist is likewise available to discuss matters with the client.

  • Record Keeping 

    I will keep information relevant  to our counselling session, including the forms you have filled in and notes for each session. These are critical to helping me keep track of our progress together and for the purposes of accountability. These notes will be stored in a private and secure location. You have the right to view the records I keep about you upon request.  I will keep all records relating to our work together for a period of at least seven years from the date of your last contact with the service. Under some circumstances I may be required to keep records for a longer period.

  • Contact between Sessions 

    Email or telephone contact will be limited to practical arrangements only, such as bookings or altering session times.  I will not enter into telephone or email counselling except by prior arrangement. If you are faced with an emergency in between sessions, please contact the appropriate emergency service. In a life threatening situation, call 000 without delay.

     

  • Ending Counselling

    In general, the counselling relationship ends by mutual agreement.  However, you have the right to end your counselling service at any time. i would appreciate you letting me know if you decide not to return to counselling, particulary if you need to cancel an appointment.  If at any time I feel that our counselling is no longer appropriate for you, I will discuss this with you, I may suggest dscontinuation or a referral to a more appropriate service.

  • Acknowledgement

    I have reviewed this Professional Counseling Informed Consent Agreement. I  understand my Client's Rights set in this form.

    I accept this agreement and consent to counseling.
