This form is intended to collect information from individuals applying for the PhilSA AD ASTRA Scholarship. All data gathered will be processed in accordance with Data Privacy Act of 2012. The information collected shall be used solely for official PhilSA scholarship-related activities.
Prior to completing this form, applicants must ensure that they have carefully read and fully understood the guidelines and requirements of the PhilSA AD ASTRA Scholarships Program. Applicants are likewise advised to prepare electronic copies, in PDF format, of all required supporting documents. Applications with incomplete documentary requirements shall not be processed.
