A GREENER FUTURE

RELEASE OF LIABILITY, WAIVER OF ALL POSSIBLE CLAIMS AND ASSUMPTION OF RISK AGREEMENT. PLEASE READ CAREFULLY.

By signing this you are waiving your legal rights and you give up the right to sue. If the Participant is under the of nineteen (19 I confirm that I am his or her parent or guardian and that I have executed this age Agreement on behalf of the Participant.

This Agreement will be binding on myself or my child or ward, as the case may be, and our respective representatives, heirs and assigns.

As a participant in events hosted by A Greener Future, I have read, understand and, by signing this form, agree to the following terms:

Release Form:

I hereby agree to release A Greener Future, its representatives, agents, servants and employees from liability for any injury to the named person resulting from any cause whatsoever occurring to the named person at any time while participating in litter cleanups and related activities, including travel to and from the event.

I acknowledge that accidents can occur with or without any fault on the part of the Participant, A Greener Future or the Agents and that participation in the event is at the Participant's own risk.

In the event of any accident or illness affecting the Participant, I authorize A Greener Future and the Agents to authorize, on my behalf, all procedures, including admission to hospital and any necessary treatment deemed essential for the care and well being of the Participant. Such action is only to be taken when immediate authorization from the undersigned cannot be obtained.

I agree to indemnify and hold harmless the releasee from any and all liability for any damage to the property of or personal injury to any third party resulting from my participation in the event and any or all related activities.

I have read and understand this agreement and I am aware that by signing this agreement, from this day forward I am waiving certain legal rights which I. my child, my heirs, next of kin, executors, administrators, assigns and/or representatives may have against the releasee.
