Important Notes
1. This is the form for younger children. 7th through 12 graders can sign up to attend Teen Leadership Camp and volunteer for Daring Disciples Camp using this link. https://form.jotform.com/240444668571058
2. This form will allow you to add up to five children to attend Daring Disciples Camp. If you would like to submit this form for six or more children, please contact Michelle at youthministry@bellarminechapel.org before beginning the form. For any other questions, email Pam at sacraments@bellarminechapel.org.
3. If you have previously filled out the comprehensive yearly registration form, you do not need to duplicate the details and medical release, but we do need confirmation of which of your children are registering for this year's camp.
