As discussed more fully in its Harassment Policy, Tembo Hospitality Group is committed to providing a work environment that is free of discrimination and harassment. If you believe that you have experienced or have observed inappropriate workplace behavior, please fill out this form and return it to your supervisor and/or the Human Resources Department. Attach additional files as needed. Please complete this form to the best of your ability so that Tembo Hospitality Group can thoroughly investigate complaints and resolve issues. Of course, you are welcome to contact Tembo Hospitality Group by phone to provide any relevant information.

To investigate your complaint, Tembo Hospitality Group will need to follow up with you, with the persons alleged to have engaged in misconduct, and any witness(es) with knowledge of your complaint. The company will do its best to discuss this only with those persons with a need to know and will take action to address any substantiated acts of harassment or discrimination, up to and including, termination of employment. By completing this form, you hereby acknowledge that the information provided in this complaint is true and correct to the best of your knowledge. Please also be advised that Tembo Hospitality Group will not retaliate against you or any witness for participating in any good-faith complaint. Any acts of retaliation should be promptly reported to your supervisor or to Tembo Hospitality Group.
