USE AND REUSE AND WAIVER OF LIABILITY AND RELEASE for Radio, Television, Video, Print and Other Publication or Media (including the Internet)

I give EMO the rights to use and reuse my name and/or image including photographs, video footage, videotapes, films and other reproductions or depictions of me in any and all publicity or media, including derivative works of the foregoing, in any way deemed appropriate by EMO. I release EMO, its successors, assignees and licensees from any claim for invasion of privacy, right of publicity, defamation or any other claim of any nature arising from the exercise of the foregoing rights.

CRIMINAL BACKGROUND CHECK AND REFERENCE POLICY

To insure the protection of clients, employees, volunteers and programs, Ecumenical Ministries of Oregon will perform background and criminal history checks on each potential volunteer. Although a criminal record is not necessarily disqualifying, eligibility for and continuation of volunteer status will be contingent on satisfactory results of these procedures. Each individual progtam will also require potential volunteers to provide references in accordance with that program's policies and procedures.

CONFIDENTIALITY POLICY

All client information obtained while performing the duties of any assignment at any program of EMO, including client name, information and proprietary business information, is confidential and cannot be disclosed to individuals not affiliated with EMO. In addition, each individual program may have a detailed confidentiality policy that must be adhered to by volunteers for that program. The confidentiality agreement continues even when the volunteer no longer is a volunteer at EMO. 

LIABILITY POLICY

Service as an EMO volunteer may involve certain risks. These may include, but are not limited to, risk of illness or injury. Volunteers are responsible for investigating the risks that might be encountered at each program site for which they undertake volunteer service and for taking appropriate steps to minimize these risks. Volunteers may not hold EMO, its programs or its employees liable for any illness or injury that may occur as a result of their volunteer service.

I have read the above policies on Criminal Background Checks and References, Confidentiality, and Liability, and I understand and agree to the policies, procedures, terms, and conditions stated therein. I authorize the investigation of all matters which EMO deems relevation to my background and qualifications, including all information given in this Volunteer Information Forms and in any attachments, supplemental applications or interviews. I authorize you to request and receive such information, and I release from all liability any persons, employers or other entities supplying it. I also release you from all liability which might result from making the investigation.

I further certify that:

• All the information I have provided is true and complete to the best of my knowledge.
• I understand that my eligibility for, and continuation of, volunteer status is contingent upon satisfactory results of the stated identification verification procedures.
• I understand and agree to adhere to the stated confidentiality policy.
• I agree not to hold Ecumenical Ministries, its programs or employees liable for any illness or injury that may occur asa result of my volunteer service.
• I understand that my volunteer service may be terminated at any time at the request of myself, the Volunteer Coordinator of my program site, or the Program Manager of my program site.
