Massage Consent Form for Minors
Before providing massage therapy to minors (defined as anyone under the age of 18, unless otherwise emancipated), consent from a parent or legal guardian must be obtained. For clients age 12 and under, the parent/guardian is encouraged to be present in the treatment room during each session, or until the client feels comfortable. For clients ages 13-17, it is up to both the minor and their parent/guardian whether they remain alone in the session room with the massage therapist. Appropriate draping will be used at all times during the massage and clients will only be asked to undress to a level that is comfortable.
By signing below, I acknowledge that I am the parent or legal guardian of the minor listed, I have read the guidelines provided, and I consent to the minor in my care receiving therapeutic massage.
