Sign this waiver and submit ONLY if you do NOT want your child photographed or in any short videos that may be taken. Images are used for advertising CISF programs, activities, event, workshops, and, at times, for grant reporting when required.
By signing below, I acknowledge that my child may NOT be photographed, filmed or video-taped while participating in CISF activities, programs, events, or workshops. I hereby unconditionally do NOT authorize Conanicut Island Sailing Foundation to use any such photographs or videos of my child/children for fundraising, advertising, brochures, website promotion, promotional flyers, camp promotion and/or other public relations and advertising medium.
