SYSA COACHES INFORMATION/CODE OF ONDUCT/BACKGROUND FORM

All coaches will present themselves in the highest moral character at all times. The following behavior will NOT be tolerated: 1.Disruptive behavior, lewd, vulgar or abusive language towards parents, coaches, officials, team players or SYSA board members at any SYSA event. 2. Physical violence or threats of physical violence. 3. Smoking or tobacco use while at any SYSA team function. 4. Drugs (illegal or prescription) and/or alcohol or being under the influence of drugs or alcohol while at any SYSA team function. Any coach who violates the CODE OF CONDUCT will be immediately suspended from their team for the remainder of the season. Any coach who has been suspended has the right to appeal his/her suspension at any regularly scheduled or special called meeting of the SYSA Board of Directors. The SYSA Board is under no obligation to call a special meeting as a result of a coaches' suspension. Board meetings are held every 1st and 3rd Wednesday of the month at 7:00pm at 500 May Road, Seagoville, TX 75159. For any other questions or concerns call 972-287-7580.

Iunderstand an investigative consumer report may be requested with respect to my application for SYSA. I also understand requests may be made from public and private sources including but not limited to: criminal and/or civil records. My signature below hereby fully releases and holds harmless SYSA from any damages resulting from theinformation being provided. I hereby authorize, without reservations, law enforcement agency provider contacted by SYSA to furnish any and all information as described above. I further authorize without reservations ongoing procurement of all reports described above during my contract.

ACOPY OF YOUR VALID DRIVERS LICENSE AND A $10 NON-REFUNDABLE ANNUAL APPLICATION CHARGE IS REQUIRED TO BE TURNED IN WITH THIS APPLICATION.

*The above information is required by law enforcement agencies and other entities for identification matching when checking records. It is strictly confidential and is used for background screening purposes. Upon approval all coaches will be issued an ID badge exclusively for their personal use throughout the season. Each coach will be fully responsible for their ID badge until the end of the season.
