Educational Scholarship Application Form
SCHOLARSHIP CRITERIA

High School, Pre-University, Secondary School Students
- Applicant must have a minimum of a 3.0 GPA
- Applicant must submit certified summary of his/her transcript
- Applicant must have attended Dooly County High for a minimum of 2 years and will graduate from DCHS within the year of the application
All Students
- Submit one completed scholarship application
- Submit a 500 word essay
- Have two letters of recommendation sent separately be recommender
- Must include the Annual Scholarship Award Report with application materials if reapplying. The application must show new activities from which a previous award was received
- Incomplete applications will not be considered
- All application materials must arrive by close of business (Eastern Standard) on Tuesday, April 30, 2026. Materials arriving after the deadline will not be considered.
- ***Should be a high school senior with plans to attend a two-year or four-year college or university
ESSAY REQUIREMENT
The essay must be typed (if uploaded via the online form) or typed in the space provided.

In 500 words or less, describe your proposed course of study (thesis, etc.) related. Relation to these areas should be directly shown. Describe your career objectives andhow the award of an AH4D Scholarship would help you attain them. Comment on what you have acheived and learned through your studies and activities (i.e. mentoring Program).

LETTERS OF RECOMMENDATION
Two (2) letters of recommendation are required. At least one of the letters should be from a faculty member who is familiar with your educational program. Faculty recommendations must be on instituation letterhead. References should comment on your character and potential for leadership and/or making a significant contribution to your future profession and/or community. This can be based on specific examples drawn from your course work, projects or activities. Each letter of recommendation must be signed and sent directly to AH4D by the person making the recommendation.

DEADLINE
***The application must be submitted via the online form or by mail by Tuesday, April 30th, 2026 at 5:00 PM EST. The transcript, letters of recommendation (and paper application if you wish) must arrive by the deadline and mailed to the below address. Late/incomplete applications will not be accepted.
