2023-2024 Re-Registration Agreement (Revised January 25, 2023)

Dear Parents/Guardians,

We are thankful that you have chosen Hunting Park Christian Academy for your child(ren)! Below you will find the re-registration form for the 2023-2024 school year. The information below includes a re-registration timeline and fee schedule, a note about our chaperone policy, and a few requests for permission/release. We are happy to announce that for the 10th straight year our registration fees will remain the same! And for the 5th straight year, the parent portion of tuition will also remain the same!

We are also happy to provide phone support to anyone seeking assistance to complete these forms online. Please call the Main Office (267-331-8002) to schedule a day and time.

Due by Friday, February 10, 2023*

I understand that to reserve a spot for your child for next school year, this form along with payment must be submitted no later than Friday, February 10th.Should I wish to enroll new siblings, I will enter their name and grade below and call the office for a new student application. I understand that this does not guarantee a spot for my new student.I understand that there are 3 reasons why a spot will not be reserved for my child(ren) for the 2023-2024 school year: 1) If I do not turn in the form and payment, 2) If my tuition status is not current and up to date, and 3) If my student(s) are among those who do not pass and their spot gets filled. (Please call the Main Office to ask.)I understand that the fee for registration is $150 per family; Book/FACTS (formerly RenWeb) fees are $65 per student. Both fees must be paid together, and all registration fees are nonrefundable. (Payments may be made online or in-person in the Main Office.)

If I plan to apply to HPCA for a tuition scholarship, the school requests that each family apply first to the Children’s Scholarship Fund. This is a lottery-based scholarship that awards scholarships over a four-year period. Many HPCA families have received awards over the past 11 years. I can apply online at https://www.csfphiladelphia.org/scholarships/apply/. There are only a few requirements, and the application is short, but I must apply before March 1st. If I need assistance, I may contact the Main Office.
Due by May 12, 2023*

I understand that every student’s tuition is a shared investment funded by both parents and HPCA’s supporters which include individuals, churches and foundations. Once all household proof of income for the entire household is submitted (Due May 12th), a tuition grant will be given to each family that will lower a family’s portion to $2,400 for the first child, $1,600 for the second, and $1,100 for each additional child (paid over 10 months).To receive this grant, I understand that I must submit proof of income (i.e. 1040 tax return, SSI, and/or assistance paperwork.) And that if no proof of income is submitted, parents will be responsible for the entirety of the tuition amount. (Note: We cannot accept W-2’s, paystubs, bank statements, etc.)I understand that the first tuition payment is due by August 20, 2023, and that monthly tuition payments are due by the 20th of each month (August-May).

If I am interested AND I qualify for a tuition scholarship, I understand that I may request an application from the Main Office and must complete it by May 31st for consideration.

Parent Volunteers and Chaperones
“According to the new state laws enacted for August of 2015, parent chaperones are now required to get clearances before being allowed to chaperone a field trip. These clearances are good for 5 years. If a parent has resided in Pennsylvania for 10 consecutive years, only the criminal and child abuse clearances are required and they are free. If a parent lived in a different state during that time, an FBI Clearance with fingerprints is required and will cost $20-30. Parents who would like to chaperone trips can apply for the certifications in the office or at home, and they must be submitted to the main office prior to parent participation on any trip.”*

I have reviewed the school’s official clearance policy for all volunteers with direct contact of students, as stated above, and I understand that parents are responsible for any associated fees.I understand that for criminal clearances, I may apply at https://epatch.state.pa.us/Home.jsp.I understand that for child abuse clearances, I may apply at https://www.compass.state.pa.us/cwis/public/home
Request for Permission and Release - Photos and Videos
Occasionally, Hunting Park Christian Academy may take photographs/videos in classrooms for use in art projects, room displays, the school newsletter, brochures and promotional materials. This permission will remain in effect until the full consent is withdrawn. You may withdraw your consent at any time by notifying the school in writing.

I give permission to HPCA to use the first name, grade and any photograph/video images of my student(s) for educational and promotional purposes.In addition, I agree to release and hold harmless Hunting Park Christian Academy, its Board members, agents, officers, contractors, volunteers, and employees from and against any and all claims, demands, actions, complaints, suits or other forms of liability that shall arise out of or by reason of, or be caused by the use of this student’s name or image.
Request for Permission and Release - Short Walk
Occasionally, teachers take their students for short walks off school property. They may go for a walk looking for certain types of leaves in the fall or to the community garden as part of a school assignment. You will not need to sign a form for each walk and you may withdraw your consent at any time by notifying the school in writing.

I give permission for my student(s) to go on short walks/trips with his/her teacher and class for educational purposes.
