REQUEST FOR DENTAL CONSULTATION

If you are experiencing an emergency, please contact your primary veterinarian.

We believe the health and well-being of your pet, family, and our team members are a top priority. 

Please read the following in regards to our processes.

• A Healthy Pet Exam deposit ($40) is required for each pet.

Please note: To secure your appointment spot, the PAWS Cares Community Veterinary Clinic requires a $40 non-refundable deposit that will go towards your total bill at checkout.

All pets will recieve an exam from one of the our veterinarians and if necessary a follow-up care plan will be made. 

Reschedules and cancellations must be made at least 72 hours prior to your scheduled appointment time. If you fail to do so you will forfeit your deposit.

PAWS reserves the right to refuse service to animals we believe are generally unhealthy, possess medical concerns, aggressive or pose a risk to our staff, and/or other animals. Please see your regular veterinarian for any illnesses prior to coming for vaccines. PAWS will not vaccinate animals who are vomiting or have bloody diarrhea.

If you have any medical records, please bring them with you.

PAWS can only accept cash and credit/card payments.

The PAWS Cares Commuinity Veterinary Clinic can assist with basic pet needs, however, we are not a replacement for a full-service veterinary hospital.

Waiver: I, as the owner or authorized agent of the pet(s) list with this form, verify that the above information is correct and that I will be financially responsible for all charges or services provided by PAWS and understand that payment is due at the time of service. I hereby covenant to indemnify and hold PAWS and its staff harmless from and against any and all claims, actions, causes of action, damages, losses, costs, liabilities, charges, fees (including attorney’s fees), and court costs claimed as a result (either directly or indirectly) of the care, custody, or handling of my animal and covenants not to sue PAWS for any matter regarding the care, custody, or handling of my animal, excepting only the gross negligence or intentional torts of PAWS. I am at least 18 years old and have the authority to make decisions regarding this animal(s). To the best of my knowledge, this animal(s) is healthy, not pregnant and can receive medical services. I do not hold PAWS liable for any adverse reactions or complications following services received.
