I authorize the performance of repairs on the vehicle identified on this authorization form (the vehicle) and the use of the parts for repairs. I grant Froggy Auto Wraps and-it's employee's permission to operate the vehicle for the purpose of testing and inspection.
Additional hidden damages may be found upondisassembly of the vehicle and may require a supplement. Our policy is to discard all parts unless instructed otherwise. I expressly acknowledge mechanics lien on the vehicle to secure Pavment for the parts and labor to repair the vehicle.
I understand that there are fees due if the vehicle is not repaired with Froggy Auto Wraps , an administrative fee of $500 (five hundred dollars), a storage fee of $50 (fifty dollars) per day, and any additional services performed on the vehicle must be paid to release it. Froggy Auto Wraps is not responsible for loss or damage to the vehicle or for articles left in the vehicle in case of fire, theft, or any other cause beyond our-control. Acknowledge that I am responsible for payment for parts and labor to the repaired vehicle.
I acknowledge Froggy Auto Wraps has informed me that repairs will be performed as per agreement in lieu of deductible and/or payment of full insurance estimate.
