In regard to the possible threat of Covid-19;

I further acknowledge Eden West Gourmet can not guarantee that I will not become infected with the Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, store staff, and other store clients and their families.
I attest that I will not attend an event if I experience any of the following: Any symptom of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.
Cancelation policy:

I understand that to obtain a full refund, I need to contact the store by email or phone at least 3 days before the start of the class.
I understand that if I cancel after the above mentioned deadline, I will not get a refund, even partial, a credit or transfer to another event. I can however send another guest in my place. I will also make sure to contact Eden West Gourmet with the details of that new guest prior to the start of the event.
Other liabilities:

I understand that different types of allergens may be used during cooking classes and for other purposes in the facilities. Ingredients can not be substituted in the recipes offered during the classes. I therefor accept the risk of being exposed to possible food allergens and make myself aware prior to the class start if any concerns, by contacting the store.
I hereby release and agree to hold Eden West Gourmet harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the store, or that may otherwise arise in any way in connection with any services received from Eden West Gourmet.
I understand that this release discharges Eden West Gourmet from any liability or claim that I, my heirs, or any personal representatives may have against the store with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from Eden West Gourmet..
This liability waiver and release extend to Eden West Gourmet, together with the owner, contracted Chefs, and employees
Eden West Gourmet will keep this waiver on file and will contact you if any important information needs to be shared after the event. Information provided on this form will not be used for promotional purposes or shared with any other parties.
