Please submit this form directly to the Saskatchewan Nurses Foundation by clicking the submit button at the end. The application form and all necessary information must be received by September 30 of the year of application. Applications for which references or other documentation are not received by September 30 will not be considered. There is additional information provided for some items by hovering over the section of the form. In addition, the Application Guidelines will identify policies related to SNF bursaries and provide examples of acceptance to program and tuition statement documentation to be submitted from Saskatchewan education institutions.

Just submit the statement(s) for any payments you have made. If you receive a bursary and additional tuition statements are needed, you will be contacted by email. Direct your referees to the appropriate reference form (employer or general reference) available on the website and ask that they complete it and submit it directly to the Foundation using the submit button on the form. Remind them that references must be submitted by Sept 30, 2023 in order for your application to be considered. You are responsible to ensure that all references have been submitted to the Foundation by the application deadline.
