We are qualified to accept both types of vouchers. Vouchers may not be used for overdue fees. Vouchers are not instant payment, they can take 6 to 8 weeks to be applied to you invoices

During class and over the dance year, including performances & events, photos and video footage will be taken of our dancers by our staff.

By signing and submitting this form you agree to this footage being used only for Studio 5 School Of Dance , for the purpose of advertising and promtional materials.

Please Note - Photographers commisioned by Studio 5 are not included and permission for these photo shoots will be requested separately. Permission for these third parties to use this footage for promotional purposes will be requested at the time of the shoot also.

We are a friendly, inclusive and kind studio. Rude parents/students or any type of abuse means you will be asked to leave & your dancer removed from our classes.

All classes are closed to spectators for the safety and comfort of our dancers and teachers.

Dancers are expected to be in Studio 5 uniform in class, with hair tidy, minimal jewellery. A fresh drink bottle and a healthy snack. (pleae no popcorn, soft drinks or gum)

Smoking is not permitted on any NSW Government property.

Invoices are provided Termly. Payment is due 1 week after invoice is sent. If you need a little extra time, just please let me know. Unpaid depts will be sent to a collections agency.

Our Show is held in December. As per your booklet , there is a cost involved for costumes. Once your child has been measured and costumes ordered for this event, If you choose to not participate in this event you will be responsible for these costs. You can notify your teacher at class if you are not particpating in this event. Show preparation begins Term 2.
