She Plays Soccer Academy is excited to announce our Spring Break Camp for kids ages 5-12! From March 20th-24th, campers can develop their technical skills on the turf field at Van Tech High School. Each day from 9:00 am-12:00 pm, campers will participate in fun gameplay and drills designed to improve their ball handling, passing, and shooting. Our experienced and certified female coaches will provide individual attention to help each camper reach their full potential.Whether your child is a beginner or an experienced player, this camp is the perfect way to stay active and improve their soccer skills during the break. Our camp is designed to empower and inspire young girls to pursue their passion for soccer. With experienced and certified female coaches leading the program, campers will be exposed to influential role models who inspire and encourage them to reach their full potential. Informed Consent and Acknowledgement I hereby give my approval for my child’s participation in any and all activities prepared by She Plays Soccer Academy during the selected camp. In exchange for the acceptance of said child’s candidacy by She Plays Soccer Academy, I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless She Plays Soccer Academy and all its respective officers, agents, and representatives from any and all liability for injuries to said child arising out of traveling to, participating in, or returning from selected camp sessions. In case of injury to said child, I hereby waive all claims against She Plays Soccer Academy including all coaches and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event. There is a risk of being injured that is inherent in all sports activities, including soccer.
Medical Release and Authorization As Parent and/or Guardian of the named athlete, I hereby authorize the diagnosis and treatment by a qualified and licensed medical professional, of the minor child, in the event of a medical emergency, which in the opinion of the attending medical professional, requires immediate attention to prevent further endangerment of the minor’s life, physical disfigurement, physical impairment, or other undue pain, suffering or discomfort, if delayed. Permission is hereby granted to the attending physician to proceed with any medical or minor surgical treatment, x-ray examination and immunizations for the named athlete. In the event of an emergency arising out of serious illness, the need for major surgery, or significant accidental injury, I understand that every attempt will be made by the attending physician to contact me in the most expeditious way possible. This authorization is granted only after a reasonable effort has been made to reach me. Permission is also granted to the She Plays Soccer Academy and its affiliates including Directors, Coaches, and Team Parents to provide the needed emergency treatment prior to the child’s admission to the medical facility. Release authorized on the dates and/or duration of the registered season. This release is authorized and executed of my own free will, with the sole purpose of authorizing medical treatment under emergency circumstances, for the protection of life and limb of the named minor child, in my absence.
Refund Policy for She Plays Soccer Academy: At She Plays Soccer Academy, we understand that plans can change and unexpected events can arise. We want to be as accommodating as possible to our customers, while also protecting the integrity of our programs. Therefore, we have a refund policy in place to accommodate those situations. Refunds will be granted for the full amount of the program fee minus a $50 administrative fee if requested 15 days or more before the start date of the program.No refunds will be granted for requests made less than 15 days before the start date of the program. Please note that refunds may take up to 14 business days to process and will be issued to the original payment method.
