I recognize that certain inherent risks are associated with the service (s) in which I am participating and I hereby assume full responsibility for personal injury to myself and any personal property damage that may occur. This includes but is not limited to any risks that may arise from negligence or carelessness on the part of the persons or entities being released from dangerous or defective equipment or property owned, maintained, or controlled by them, or because of their possible liability without fault.

III. MATERIALS.

I understand that permanent jewelry is not indestructible. Like any other piece of jewelry, normal wear, and tear will occur, not due to defective or low-quality metals. I understand my jewelry can stretch and or break if snagged. Proper care should be given to maintain the integrity of jewelry, e.g. avoid prolonged exposure to hot tubs, swimming pools with high or unbalanced chlorine levels, and/or harsh cleaning products; rinse and dry after any exposure. Perfumes and lotions should be wiped from jewelry to maintain the integrity of the metal. Chains and charms must be cleaned with mild soap and polished with a jewelry cloth in order to remove oxidation. In rare occasions, a person's skin may reject or react to chosen metal. I understand that ALL SALES ARE FINAL. NO REFUNDS OR EXCHANGES.
