If awarded the scholarship, I will provide proof of enrollment (copy of official class schedule, registration bill, or tuition receipt) 30 days prior to distribution of scholarship funds. If my circumstances change and I am no longer eligible, I will contact the RISE Project scholarship committee chair (see below) and inform them of the change.
