FYI... You MUST register for the camp of your true age during the camp; anyone that registers outside of their age group will have their registration rejected and refunded and you risk missing out because this camp sells out very quickly.  Therefore, once the camp is "SOLD OUT" we cannot accept any further registrations.  If you would like to be put on our WAITING LIST for the camp (if sold out), please email us at admin@zprofutbol.com with your information. 

(1) INFORMED CONSENT & DISCLAIMER:

During any training camp or clinic, every effort will be made to assure your safety.  However, as with any training program, there are risks.  These risks may include, but not limited to, the chance for musculoskeletal injuries & stress on the cardio-respiratory systems.  By approving this consent & disclaimer, you assume the responsibility for these risks and waive any possibility for personal damages.  You also agree that, to your knowledge, you have no limiting physical condition or disability that would preclude you from this training program.  If you do have any medical issues or condition that may be of concern, you further agree that a physician has cleared you to participate in this type of training program.   Participants who do not have prior physician clearance MUST acknowledge they have been informed of its importance.  By consenting below, you accept full responsibility for your own (athlete’s) health & well-being and you acknowledge an understanding that the leaders of this program, as well as the facility administration (i.e. Colorado Academy), assume no responsibility. 

Please note that the training coaches are NOT in control of the training facility and grounds in which trainings are held and, therefore, cannot be held responsible for any incidents within the facility or on the grounds. However, all coaches will do what is possible to maintain safety within their training area(s). 

(2) CONSENT FOR PHOTOGRAPH & RECORDING PARTICIPATION:

I am aware that there may be photographs and/or recording during any training session for later use on items including, but not limited to, websites, promotional materials, social media (i.e. Facebook, Twitter, etc.), posters, flyers, and any other media available, or in any other way they see fit for professional use. 

By approving this document, I hereby authorize use of my [goalkeeper] name and likeness, voice, verbal statement, and filmed media (photos or video) for any of the aforementioned purposes.

ATTENTION PARENTS / GUARDIANS

Rules of the Field:  Please understand that parents or guardians are not allowed ON THE FIELD during training sessions.  This is for the overall safety of our students.  Distractions increase the potential for injuries to occur so we need the students' full attention and to not have this added distraction during training.  (There will be plenty of room on the sidelines for parents to observe and/or take pictures.)

Any parent on the field [during training] will be asked to move back out of the training area.  If there are repeat occurances, the parent [and student] may be asked to leave the training without any refund. 

We sincerely appreciate your understanding and cooperation.  The safety of our students is a top priority in all of our training sessions.
