By signing this form, you acknowledge, represent, and agree that:

-All information provided on this form is true, complete, and accurate to the best of your knowledge at the time of submission.
-You have not knowingly provided any false, misleading, or incomplete information.
-Upon acceptance of your surrender request by Touching Hearts Animal Rescue, all legal right, title, and ownership of the cat shall immediately and irrevocably transfer to Touching Hearts Animal Rescue.
-Once ownership has transferred, you relinquish all rights to reclaim the cat. If you later wish to regain custody of the cat, you must submit an adoption application and successfully complete the same adoption process required of all members of the general public, with no guarantee of approval.
-Any applicable surrender fees must be paid in full prior to the cat being accepted into the care of Touching Hearts Animal Rescue. Failure to pay the required surrender fee(s) shall result in automatic termination of the approved surrender request.
