APPLICANT REQUIREMENTS
Minimum Requirements
- Candidates must be an associate of an URPT partner clinic upon the time of matriculation and have graduated from an accredited OT or PT program
- Current PT or OT state licensure, with modalities certification (if applicable)
- 6 months experience, completed EMR training and therapist onboarding programs and ability to carry caseload of 12+ clients per 8-hour day
- CHT Mentor of same profession within the company and within a reasonable distance for weekly or bi-weekly travel
- Active ASHT membership required prior to introductory course material; AOTA/APTA recommended
- Regular access to personal computer with Gmail, company email and use of Outlook calendar
- THREE Letters of Reference (peer or mentor therapist, Clinic Director/Manager and Regional Vice President)
Above Requirements + Preferred Qualifications
- 12 months experience in Hand/UE Rehabilitation in an outpatient orthopedic setting
- URI Intro to Hand Therapy and Intro to Orthoses coursework (or comparable external to URPT)
- Ability to ultimately carry a 40% hand/UE caseload during Fellowship
*ASHT Membership is required due to the significant resources found through this organization that are utilized during the program. Membership must be active prior to program matriculation.
*Finalists will be interviewed on a rolling basis upon full completion of the application. Cohort seats are limited to a maximum of ten (10) due to restricted travel and resources. Program acceptance will be prioritized and limited to those who meet criteria and have an appropriate regional mentor available, with hand therapy growth plan approved by RVP, per policies and procedures. For those selected, pre-program introductory online modules will be assigned and are to be completed before program matriculation.
*Tuition is shared between Upstream Rehabilitation and Fellow.
