Purpose

The Niagara Ontario Health Team, Dementia Care working group would like to hear from you about your experience with accessing comprehensive assessments for cognitive changes. In this focus group, we will discuss your experience with getting an assessment, successes and barriers to accessing support from your medical team and community support.
