    If you are only enquiring whether we can accommodate you for lessons, then ensure you tick the box that asks this question.

    Please fill in all sections, whether you are enquiring or requesting to sign up with us, and we will respond accordingly to your enquiry or whether we can accommodate you or not as a student.

    By completing the form fully, it will save time on your first lesson with filling in forms and allow us to have all the details we require to get you set up on our computer system and send details by text to you so you can download our pupil app.

    When we send the link to the Pupil App, please note that the code supplied in the text to download the App only lasts for 24 hours. If you have not downloaded the app within this time, please message us, and an additional code can be sent.

     

    Please note we have a limited catchment area for our students; however, we will travel further if you specifically require our services, but this will be reflected in our prices to cover additional fuel and travel time. You can request what those charges may be prior to accepting our services.

    On your first lesson, we require you to bring your provisional licence as we must physically see it before allowing you to drive.

     

    All our lessons are booked for you on the basis of pre-payment.

    This can be 24 hours before the lesson begins. If paying by cash on the day of the lesson, this must be paid at the start of the lesson.

    If you do not have the money and need to go to a cash point, then we will do this at the start of the lesson and not when the lesson ends. This will prevent us from being late for our next pupil.

    To qualify for a block booking discount, you must pay for the block booking at the time of booking the block. (block bookings are a minimum 10 hours)

    We prefer bank transfer as this gives us a financial footprint of all transactions.

    All your payments will show up in your pupil app so you can keep track of payments made.

    Once you are booked onto our system you will receive a text message to download our pupil app. The code for login lasts for 24hrs but we can issue you another if you have not downloaded the app in time.

    On the home page of the pupil app is a tab called "resources" clicking on this tab will take you to a page with lots of information regarding things to do with driving, and also has the details for bank transfers.

    The General Data Protection Regulation

    Access to the personal data we may hold about you.

    As a client of ours, we have to ask for certain information about you such as:

    Name, Address, Phone number, Driving licence number & National insurance number (to confirm your licence), Date of birth, Gender, email address, etc. 

    Clients can view and edit any data we hold about them at any time free of charge.

    If requested, we will provide you with a readable copy of the personal data that we keep about you, almost instantaneously via the database that we carry with us at all times -  We will provide the information without any charge. Should you wish a printable copy we can send this by email if requested.

    We allow you to challenge the data that we hold about you and where appropriate, you may have the data: erased, rectified, or amended.

    The Information we hold on you is purely used in-house and is not distributed to any third party.

    The information is always collected initially via this portal, a paper format is then printed and stored securely and is then transferred to a digital database stored via a secure cloud-based software. The Paper version is then only accessed during your lessons and when not in use is kept securely locked away.

    When you cease to be a client of ours either by discontinuing lessons or by passing your driving test, you can ask to have your details removed from my system, this will be carried out within 24 hours. However, certain parts of information such as name and address may still be required for accounting purposes but will be deleted once 6 years have elapsed, this is so we comply with record-keeping for the HMRC.

    If you do not ask for the information to be deleted, it will remain on our system for the basis of accounting and statistics purposes only.

    All our vehicles have CCTV recording internally and externally of the vehicle. No internal footage will be used without your consent on any social media platform or our website.

    We like to use photographs of pupils when they become “Full licence holders”  as an advertising source and to boast about their success. If you are not happy with us taking a picture after passing please let us know. Images may appear on our website and other social media pages that we have.

     

    I agree / disagree to Life Skills Driving School holding data as mentioned above on a database as long as it is not used by third parties.

    Terms and Conditions

    This Agreement of tuition is made on the date when the form is submitted between.

    Life Skills Driving School

    DVSA Licence Number 33934416, and the person completing the form as submitted.

     I, the undersigned, agree that I will attend appointments booked with Life Skills Driving School for the duration of the booking allocation time and will be available promptly for the allotted booking times.

    If I require the appointment to be cancelled due to illness or prior arrangements being forgotten about, I will give a minimum of 48 hours' notification. Failure to do so will result in full payment of the booked lesson/s being made at the discretion of Life Skills Driving School.

    I understand it is a requirement of training that lesson fees are prepaid either at the beginning of each lesson or by bank transfer before the lessons. (Bank transfers are the preferred method of Life Skills Driving School, as a paper trail is made and any discrepancies are traceable. If I do not have funds to pay at the beginning of the lesson or have not paid in advance by bank transfer, please inform the instructor at the beginning of the lesson so that they can visit a cash point at the start of the lesson, not the end, when they may have another pupil to get to.

    If I regularly have not paid for lessons in advance or at the start of the lesson, then the instructor has the right to withdraw the booking in advance facility, and lessons will only be booked with a payment being made at the time of booking.

    I will ensure that I arrive for lessons in clean clothing (not muddy or a material that may damage or stain the car interior) and that I am in a fit and legal condition to drive, and for the benefit of all in the car, have considered personal hygiene so that there are no unpleasant odors in the car for the session.

    If I attend any booking in an unfit state (under the influence of Alcohol, drugs) or am on legal medication that may impair my ability to drive, I will notify my Instructor immediately. This will evoke a cancellation of the lesson, and I understand that this will be considered a late cancellation (not 48 hours' notice) and that the lesson fee will still be payable in full.

    If, for any reason, I do not notify my instructor that I am not “fit to drive” and he feels that I am under the influence of drugs or alcohol, he may cancel my appointment at any time without fees being refunded.

    Random Alcohol breath tests may occur during lessons, and I am happy to provide a specimen of breath should my instructor ask.

    I understand that mobile phones will be switched off or placed in silent mode during the duration of my tuition.

    I understand that smoking is not permitted in the car.

    Life Skills Driving School has no objection to you asking to stop at any time during your lesson (if safe to do so) for you to stretch your legs, grab a drink or something to eat, or get out and have a smoke or vape.

    I understand that from time to time my instructor may arrive late due to traffic, breakdowns, illness, or any other reason, and he will notify me as soon as practical to postpone the lesson to another date or time, or if possibl,e will add the time on at the end of the lesson.

    I understand that abusive behavior or language is not tolerated and that I must drive within the laws of the Highway Code. Speeding, aggression towards other road users, etc, may result in the lesson being cancelled. This would be with immediate effect and with a loss of the lesson fee.

    Any altercation may be reported to the police.

    I understand that CCTV is operating within the vehicle which covers the interior and exterior of the vehicle.

    This CCTV footage is data-protected and is in place to safeguard both the Pupil and Instructor and may be used on occasion as a training aid for each individual pupil of their own video footage.

    I agree to notify my instructor of any changes to my provisional/full driving licence that may require me to surrender my licence to DVLA or may have penalty points added to it.

    I agree to notify my instructor of any medical condition that may arise from time to time that may affect my ability to drive, such as impairment in eyesight, injury to arms or legs, hearing, etc, that may not allow me to control the vehicle correctly.

    I understand that my instructor may refuse use of the training vehicle for their Driving Test if they feel that I am not at a test-ready standard.

    I understand that my instructor may teach items associated with driving that may not be in the current driving test, but are considered valuable tuition for future driving, such as a turn in the road, reversing around corners, etc.

    I agree that all the information, including the driving licence information I have supplied, is true and legal information and in no way has been supplied non-legitimately.

    I understand that the fee paid for the practical driving test is a fee to DVSA and not Life Skills Driving School, if I require the use of my instructor’s car on the day of the driving test then I will pay the appropriate fee for the hours used (this is usually an hour lesson before the test and an hour duration for the test) This fee may be increased dependant on travel time, and must be paid in full before the test taking place.

    Should I have breaks in tuition, which is considered of no benefit to myself or my instructor (constantly canceling lessons or gaps in-between lessons), then my instructor reserves the right to remove me from their client list, and the pupil waiting to begin lessons may be offered the timeslot instead.

    Mitigating circumstances may prevent this from happening.

     I understand that there is a massive backlog within the DVSA booking System for practical tests and that at any stage during my training lessons may need to be canceled to accommodate another pupil who has managed to get a last-minute practical test cancellation slot.

    I understand that my instructor will give as much guidance to make me aware of where my abilities are, based on when I may be ready for the practical test. They may recommend that I book a practical test based on the current backlog, but I appreciate this is just guidance and that, nearer to the actual test date, I may have to postpone the test if my abilities are not what is required to pass the driving test. I understand that should my abilities be up to standard, then my instructor may recommend that I find a cancellation date to an earlier date.

    If I book the driving test without my instructor's consent or if my abilities are not at the required standard to pass the driving test, my instructor may refuse to take me.

    Life Skills Driving School does not teach drivers only to get them to pass the driving test; the driving test is a part of the training program, and they will cover items of driving that are no longer part of the driving test, but which are real driving skills that cover situations that may arise once you have passed your test.

    As one of our pupils, you agree that Life Skills Driving School is supplying you with training to prepare you for a lifetime of safe driving and not just to pass the driving test.
