A USTA League Team Confirmation & Agreement form is required for all USTA League teams. Captains need to complete the information listed below. Captains must digitally sign this form for the team to be included in the match schedule. Captains need to acquire confirmation from your facility prior to submitting this Agreement. The facility manager or delegated staff will send a confirming email to the League Coordinator. After confirmation, a team number will be issued to you.

Completed Team Confirmations must be submitted by February 20, 2026 to be included in the league season.

A team number will be sent to you after the Facility has also confirmed your team.

Important Dates:

Feb. 20 - Team Confirmation from Captain is due

Feb. 27 - At least 8 players registered on the team

Mar. 13 - Schedule published on TennisLink

Mar. 27 - League Begins

June 14 - League Ends

June 20-21 - Playoffs
