Please be advised that this form is not an audition form for the Certificate Program or

MOPTOP.

The Certificate Program and MOPTOP are both audition-only programs, which means that

you must first audition and be accepted, before completing this form and continuing with your

registration and enrollment. For more information on auditioning for the Certificate or Moptop

Programs, please visit our website here.

If you are an Independent Study Program applicant, or have already auditioned for and been

accepted into either the Certificate Program or MOPTOP, please continue with this form

to begin the registration and enrollment process.

Please note that as an International Student joining the 1-Month Independent Program, you are not required to obtain an F-1 student visa. However, it is important to be aware of the following:

1. Tourist Visa Requirement: Depending on your country origin, you will need to obtain a tourist visa or any other necessary visa that allows you to enter the United States. Please check with the U.S. embassy or consulate in your home country for the specific requirements.

2. Entry to the U.S.: When entering the U.S., please remember that you will not be considered a student for visa purposes. Ensure you inform immigration officials that you are entering the country for a short-term program and not as a student.

We recommend starting the tourist visa application process early to avoid any delays. If you have any questions or need further assistance, please contact us at programs@peridance.com.

your submissions!

As an international student, your Application Request Form submission must first be reviewed

by the Peridance International Admissions team. You will then be directed to the appropriate

International Student Application Portal for your application type.

You should expect to receive an email with a link to the appropriate Application Portal shortly

after submitting this form, or you will receive an email from the Admissions Team with

follow-up questions, if needed to determine your application type.
