Please be advised that this form is not an audition form for the Certificate Program or MOPTOP.

The Certificate Program and MOPTOP are both audition-only programs, which means that you must first audition for, and be accepted, before completing this form and continuing with your registration and enrollment. For information on auditioning for the Certificate Program, please visit our website HERE, and for information on audition for MOPTOP, please visit our website HERE.

If you are an Independent Study Program applicant, or have already auditioned for and been accepted into either the Certificate Program or MOPTOP, please continue with this form to begin the registration and enrollment process. If you are an Independent Study Program student who identifies as a beginner, or has less than a year of dance experience, we encourge you to contact the Independent Study Program Coordinator, Oscar Manzo, at training@peridance.com to see if the program would be a good fit, before completing this form.

IMPORTANT NOTICE

As a domestic student, you will be automatically redirected to the Domestic Student Application Portal upon submitting this form. You must complete all required forms in the Portal in order to finalize your registration and enrollment. If, for some reason, you are not automatically redirected after submitting this form, you will also receive a link to the Portal by email.

Please note that, when first attempting to access the Portal, you will be prompted to create a Jotform account. This account is 100% free and will allow you to access the Portal, save responses, and view your form submissions. To ensure that all steps in the Portal are properly completed, it is important that you always log into the Portal using the same Jotform credentials before completing any required forms.

For easy access to the Portal on your phone or tablet, please add the Portal as an "app" when prompted to on your device. This will allow you to access the Portal directly from your phone's home screen, just as you would with any other apps you already have (i.e. Uber, Instagram, etc.). For iPhone / iPad users, please click on the share button at the bottom of your browser when opening the Portal for the first time, and then click "Add to home screen". For Android users, please click the "Install App" button that appears at the bottom of the page when opening the Portal for the first time.

IMPORTANT NOTICE

As an international student, your Application Request Form submission must first be reviewed by the Peridance International Admissions team before you are directed to the appropriate International Student Application Portal for your application type. Please note that the admissions team may ask for additional evidence or explanation of your current situation to determine the appropriate pathway if it is not immediately clear which Portal is applicable. All International Student Application Portals have slightly different forms and requirements, so it is important that you use the correct one for you. For example, the International Student Applicaton Portal for new students applying for an F-1 visa will have slightly different requirements than the Application Portal for transfer students.

If your Application Request Form submission requires further follow-up, you should hear from the Peridance International Admissions team within 2 business days. Otherwise, you should expect to receive an email with a link to the appropriate Application Portal shortly after submitting this form.
