Complete the form below to RSVP for the June 28, 2023, July 22, 2023, and August 25, 2023 Assessment Days.
Please note: There is a $20.00 evaluation fee.

We appreciate that swimmers are eager for their results at the conclusion of the assessment. Results will be emailed at the end of July.

June 28 Assessments will take place at Appleby College Pool located at 540 Lakeshore Rd. W., Oakville.

July 22 Assessments will take place at Oakville Trafalgar Community Centre located at 325 Reynolds Street, Oakville.

August 25 Assessments will take place at Oakville Trafalgar Community Centre located at 325 Reynolds Street, Oakville.

I confirm that my swimmer meets the following requirements to attend the assessment:
1. Swimmer must be a minimum of 5 years old and maximum 12 years old as of the day of the tryout.
2. Swimmer must be independent and take part in practice without a parent
3. Comfortable in deep water
4. Must be able to float on front with head down and face in the water for 5 seconds
5. Must be able to float on back for 5 seconds.
6. Must be able to kick 10m on front and back starting in a floating position.
7. Must be able to jump into deep water unassisted.

OAK welcomes all swimmers to tryout for the team, however the club has limited availability for each group. Attendance at this assessment does not guarantee a position on the team.*

I accept that the tryout fee is non-refundable and I acknowledge that participating in the assessment/tryout does not guarantee a position with OAK.
By signing this form, I acknowledge that I have read, fully understand and agree to all terms and conditions. I agree to all terms and conditions in the COVID-19 Attestation and Agreement Form. I agree to all terms and conditions in the Acknowledgement and Assumption of Risks Form.*

Upon completion of this form please be sure to arrive at the pool at least 10 minutes prior to the start of the assessment that you selected above. Swimmer must have a swim suit, cap and goggles.
