Participant Committment

I understand by completing this form my child(ren) will be participating in the 2023 ADC Student Show at Orange Coast College on June 10, 2023. I further understand this annual showcase includes a Tech Rehearsal, Photo Day and stage performance at the Show which are Mandatory events if agreeing to participate.
Costumes:

I understand I will be responsible to measure my child for her/his costume, selecting the size and placing the order online (specific costume details will be provided to parents, see Avanti's website for costume instructions).

PARTICIPATION FEE is $35.00 per student which includes media download of entire Show performance and commemorative t-shirt for each dancer.

CONSENT TO CHARGE CREDIT CARD | As the parent or legal guardian of the above-named participant, I hereby give my consent to charge my credit card currently on file in my parent portal (Jackrabbit) I understand this is a minimum fee of $35.00 per dancer which is non-refundable once charged.
