I agree to hold the Episcopal Diocese of Chicago, the Church of St. Paul & the Redeemer, and any associated agencies and persons free of liability and waive any claims for payment for accident, injury, disability, or damages to the person or property of the aforementioned child(ren) arising out of or connected with their participation in any activity related to Vacation Bible Camp 2023. I give permission for photos taken of my child(ren) to be used in church-related communications. I give permission to the leaders of Vacation Bible Camp and the staff of St. Paul & the Redeemer to secure emergency medical or surgical treatment for my child(ren) and to secure routine medical care as needed if I am not present and there is insufficient time to contact me.

VBC registration fees are on a sliding scale of $50 to $250 per child. Fees can be paid online on the next page after you submit this form. Fees may also be paid by check (payable to St. Paul & the Redeemer with “VBC” in the memo line) or in person on the first day of camp using cash, check, or credit card. Please pay what you can and support SPR in keeping the magic of VBC open to all children, regardless of ability to pay.
