• Overnight Employee Check In

  • Employees must remain awake, alert, and attentive at all times while on duty, including during overnight hours. To ensure the client’s safety, employees are required to perform a visual check to verify the client’s well-being at least every two hours throughout the night.

    Definitions:  Overnight - between the hours of 12am and 5am

    Procedures:  Employees will attest that they are awake and alert by completing  this Overnight Check In form at the following intervals

    • 1am - Between 12:45am and 1:15am  

    • 3am - Between 2:45am and 3:15am

    • 5am - Between 4:45 and 5:15am

    If you are unable to complete a check in for any reason, including a technical issue with the form, you must call 267-547-0310 to check in verbally. 

    If you do not check in, we may call you to verify that you are awake.  It is very important that you answer this call.
