I agree to pay the monthly membership rate I selected above to Skin Care by Alison via auto-pay on the first of each month, charged to the credit or debit card number of my choice. My Membership Rate for the above selected treatment is equivalent to 10% off the treatment price and includes the Membership Perks listed below.

Membership Perks:

10% off all other services / treatments
10% off all products (excludes sale prices where the discount is higher than 10%)
My Membership Rate will not increase from the amount shown above while my membership is active
Option to put my appointments on auto-repeat (it’s a great way to guarantee your preferred day & time); we can easily reschedule a single appointment when the need arises
Can’t make it in that month? Gift a facial to a loved one or put the funds paid toward products or additional services. A gift certificate will be provided upon request.
Upgrade to any higher-priced service and only pay the difference

I agree to the following Terms and Conditions

Membership includes one treatment per month. The duration and type of treatment is selected above at the time of enrollment.
An initial three-month membership commitment required. Membership can be cancelled after three (3) months with written notice (email or text message is acceptable) 15-days prior to the next billing cycle. If cancellation occurs prior to completion of the three month period, the balance due is charged to the card on file and you will be issued a gift certificate for the unused portion.
A valid payment method must be kept on file. If your payment method does not go through you will be notified by the fifth of the month. If payment issues are not resolved by the 10th of the month or your appointment date, whichever occurs first, your membership is considered cancelled and you are responsible for the full price of services rendered and goods/products sold.
Late cancellation and no-shows: Our late cancellation and no show policies apply to all membership offers. If you cancel with less than 24-hours notice prior to your scheduled appointment time or do not show for your appointment, you forfeit that month’s service. Cancelling after your appointment start time is considered a no-show.
If you miss or cancel an appointment for the month, you’ll receive a gift certificate for the paid amount and you can gift your facial to a loved one or use the balance towards your next product purchase. If you send someone to take your place at your appointment, please provide 48-hours notice with their name and your consent to use your facial.
Membership fee is equivalent to the cost of the included treatment less a 10% discount.
Facial membership fee will be automatically drafted on the first of each month to the credit card number on file. You can update your number on file through the Schedulicity client portal.
Referral Rewards earned are applied to products, additional treatments, or treatment upgrades and are in addition to your 10% discount.
Promotions: If a promotional discount on a product is higher than your membership discount, you will receive the promotional discount instead of your membership discount.
When booking online, please select the Membership Facial with the appropriate time option.
Your membership status must be active to redeem membership perks.
The Membership rate at the time of your enrollment will remain the same and is exempt from price increases. If you cancel your membership and later decide to re-activate, your membership rate will be the Membership rate available at the time of reactivation.
The first auto-payment will occur on the first of the month immediately following your enrollment date.
