I hereby agree to assume all responsibility and risk from the use of riding horses while taking lessons from Ingleside Farm & Riding Academy, LLC. And further agree to hold Ingleside Farm teachers, trainers and employees free from all damages or liability for any injury to person or property arising as a result of the use of, or, lessons on said horses or equipment while staying at Ingleside Farm.

The undersigned parent(s) or guardian(s) of a student or a minor hereby give consent to any x-ray examination, anesthetic or medical or surgical diagnosis or treatment and/or hospital service that may be rendered to said minor under the general or specific instructions of any physician or hospital.

It is understood that this consent is given in advance of any specific diagnosis or treatment which may be required, but if given, to encourage the staff of Ingleside Farm, hospital staff, and such physicians to exercise their best judgment as to the requirements of such diagnosis or treatment.

The undersigned shall pay all fees for doctors, hospital, ambulances, and all other medical charges reasonable or necessarily occurred.

Under NC Law an equine activity sponsor or equine professional is not liable for an injury to or the death of a participant in equine activities resulting exclusively from the inherent risks of equine activities.  (Chapter 99E of the NC Statutes)

PAYMENT:
To secure your child's spot, a $90 non-refundable deposit or full payment is required. If you choose a deposit, the remaining balance is due one week prior to the start date of the camp. Payments can be made by check, cash or Venmo. Contact Heather at inglesidefarm@earthlink.net for details.
