Thanks for finding me, I'm looking forward to potentially working together! Please read all of the information below, fill out the entire form, and hit 'Schedule Event' in the Calendly module, and hit SUBMIT at the bottom of the jotform.
In order to secure your tattoo appointment you will need to:

1. complete this form

2. receive a confirmation email for the set appointment from my email (rosaaum@gmail.com) please note that this is different from the automated calendar email

3. and pay the $100 deposit/drawing fee via venmo @rosa-lumina

The $100 deposit will come off the total cost of your tattoo. I charge $250/hour for all custom work. For my predrawn flash designs, ask me about the price for the piece you're interested in!

We do not have a set appointment until you've received a confirmation email from me.

All deposits/drawing fees are non-refundable. If you cancel your appointment or do not show up, you will forfeit the deposit. Even if you try to book an appointment in the future.

If you need to reschedule your appointment you must do so at least 48 hours in advance for your deposit to transfer to your new appointment.
