I understand that this booking request form does not guarantee me an appointment. I understand that I will be contacted for follow up via email and will be required to send a $50 deposit in order to secure the appointment. I acknowledge my appointment isn’t secured until the deposit is received. I understand that the deposit is non refundable. If I need to reschedule my appointment I will give a minimum of 48 hours or else I forfeit my deposit. I understand that if I reschedule my appointment my deposit is transferable up to 2 months.*
