USTA Middle States & Districts
Thank you for promoting tennis in Middle States and in your local district! This application is for facilities, organizations and/or groups who hold events or unique programs that will help the sport of tennis grow. Please be detailed in your answers to the questions below, so we can best determine how we can help support you.

Please note that this is not a USTA Middle States Grant. In certain situations we may follow back up with you and suggest you apply for a grant, if a one-off sponsorship is not right for the situation.

Anyone receiving funds in this form is required to submit information to USTA Middle States after the event, recapping:
- Number of participants
- Volunteer involvement
- Follow-up plans
- General event recap
